The consultation intake checklist
Capture a complete case the first time — so reviews go smoothly and every treatment you perform is clearly supported and documented.
Identify the client & the visit
- Confirm who the client is and pull up their record (or create one).
- Note the date and the treatments planned for today.
- Check whether they’ve been treated before and what was done.
Capture the medical picture
- Allergies — including anything new since the last visit.
- Medical conditions relevant to the planned treatment.
- Contraindications to flag for the medical director.
- Current medications and supplements.
- Any prior reactions the client reports.
Document the treatment plan
- List each treatment and area you intend to perform today.
- Take clear before photos of each area.
- Add any notes the medical director should see.
Confirm coverage before you treat
- Make sure each treatment has a clear decision — approved, or covered by a valid directive.
- If the medical history changed, expect a fresh review before proceeding.
- Keep the signed directive on the record for every approved treatment.
